When selecting your RESP plan type and corresponding HRSDC Grants, the following needs to be considered :
- Number of beneficiaries (single or multiple)
- Relationship between beneficiary(s) and Subscriber(s)
- Grant type requested

Checklist to Open an Account
In order to open an RESP account and apply for any of the available HRSDC grants, the following applications are required
- Mackenzie RESP Application (Individual or Family)
- Subscriber(s) Information: Names, address, SIN, of the individual(s) who is/are opening the RESP account
- Beneficiary Designation: Name, address, SIN, relationship to the subscriber, Citizenship, custodial parent information and address.
- Investment Selection: Fund selection, Beneficiary allocation for contributions, amount, PAC instructions.
- PAC Authorization: PAC instructions and authorization (void cheque required).
- Application for Grants: Selection of the Grant requested and Selection of one fund where the Grants will be invested (only one fund is permitted).
- Subscriber(s) Signature
- Dealer Information: Dealer and Advisor information with Signature as well.
- HRSDC Grant Application Forms (SDE0069, SDE0071, SDE0073)
The following information is mandatory:
- Type of Grant requested
- Subscriber & Joint Subscriber information
- Custodial Parent information
- Beneficiary Information
- Primary Caregiver information (the Primary Caregiver is identified by Canada Revenue Agency as the Individual that receives the Canada Child Tax Benefit)
- “I do consent” box to be selected
- Subscriber(s) signature(s)
- Custodial parent signature(s)
A completed form for each beneficiary is required.
- Alberta Centennial Education Savings Grant form – ACES1
The following information is mandatory:
- Grant type to be applied for ($ 500 or $ 100 Grant)
- Beneficiary Information
- Proof of Alberta Residency (Section B)
- Enrollment at an educational Institution (only for $ 100 grants) in Section C
- Name and Signature of the Subscriber authorizing us to apply for the Grant
- Signature of the parent/legal guardian for whom identification /proof of Alberta residency has been provided.
A completed form for each beneficiary is required.
After an account is established and the first contribution or transfer into the plan is received, notification will be mailed to the Custodial Parent(s) advising them of the opening of the plan for the respective beneficiary(s).
