When selecting your RESP plan type and corresponding HRSDC Grants, the following criteria should be considered:
- Number of beneficiaries (single or multiple)
- Relationship between beneficiary(s) and subscriber(s)
- Grant type requested

Checklist to Open an Account
In order to open an RESP account and apply for any of the available HRSDC incentives, the following applications are required
- Mackenzie RESP Application (Individual or Family)
- Subscriber(s) Information:
Name Social Insurance Number Address
- Beneficiary Designation:
Name Address Social Insurance Number
Date of Birth (Helpful Hint: day and month are often inverted.) Relationship to the subscriber(s) Residency Status Custodial parent information and address
- Investment Selection:
Fund selection Beneficiary allocation for contributions PAC instructions and void cheque
- PAC Authorization: PAC instructions and authorization (void cheque required).
- Application for Grants: Select the Grants you wish to apply for and one fund where the Grants are to be invested (Please note: only one fund is permitted).
- Please ensure that Subscriber(s) sign in Section 7
- Dealer Information: Dealer and Advisor information with Signature as well.
Please Note:
In order for Mackenzie to register an Education Savings Plan with CRA, we require a signed application and valid subscriber and beneficiary information at time of setup.
- HRSDC Grant Application Forms (SDE0069, SDE0071, SDE0073)
The following information is mandatory:
- Type of Grant requested
- Subscriber information and signature
- Custodial Parent information and signature (if applicable)
- Beneficiary Information
- Primary Caregiver information is identified by Canada Revenue Agency as the Individual that receives the Canada Child Tax Benefit Helpful Hint: This is usually the mother.
- “I do consent” box must be selected in order to receive grant/bond.

The following information is mandatory:
- Grant type to be applied for ($ 500 or $ 100 Grant)
- Beneficiary Information
- Proof of Alberta Residency (Section B)
- Enrollment at an educational Institution (only for $ 100 grants) in Section C
- Name and signature of the subscriber authorizing us to apply for the Grant
- Signature of the parent/legal guardian for whom identification /proof of Alberta residency has been provided.
Please note: A completed form for each beneficiary is required.
Once an account has been established and the first contribution or transfer has been received, notification will be mailed to the Custodial Parent(s) advising them that an RESP has been opened for the respective beneficiary(ies).
